Network Maintenance Request Form

If you are having trouble connecting to the internet because of a network jack problem or a Clean Access issue, please fill out this form.  This will automatically contact someone to help you.

In order for IS&T to better serve you, we need you to complete these simple tests. After testing, if you have established that your network jack or Clean Access is malfunctioning, we will make arrangements with you to repair it.

HAVE YOU?

  • Verified your user account is enabled and your password works?

  • Made sure that your Ethernet card is seated/installed correctly in your computer?

  • Taken your computer to a room where you know someone else is getting online without any problems and tested your computer there?

  • Tested your computer with someone else's network cable that they know works?

  • Installed Clean Access? Reinstalled Clean Access? Upgraded Clean Access?

  • Completed all updates for your computer such as Anti-Virus and Windows?

  • Checked the Clean Access FAQ for your problem?

If you have completed these tests and cannot connect in your room, please provide all of the information below and return this form to the Help Desk. We will contact you to set up an appointment to troubleshoot your network connectivity issue  If you have any further questions, please contact the Help Desk at ext. 8027.

Clean Access Issues

  1. Personal Firewalls such as Norton Internet Security Suite and Zone Alarm can block the Clean Access Agent from communicating with the network. To fix this you will need to change the settings to allow the Clean Access Agent to communicate.

  2. It is normal to have to reboot several times during Windows Updates due to the fact that some updates have other updates as pre-requisites.
